ACM Insurance Release Forms?
I heard a rumor that everybody has to sign an insurance release forms prior to ACM. If this is true it sucks. But at the same time maybe it could save a lot of time if the DOD posted the form here and we all could just bring the filled out forms to the Pile.
I have an old release waiver of liability and assumption of all risk of personal bodily injury and or death for the privilege to use a trampoline and I can just cross out "a trampoline" and write in "dirt".
Also, I'll be sure and bring my OLCC card in case we need somebody to legally serve beer.
It's true...and it sucks. We'll have a "once per (calendar year) year option" available. I can't promise it will be available online before ACM, but that has been my plan.
No quicker way to kill a party than to bring up insurance. It's like someone reminding us that were not sixteen any more.
Bring it on, hopefully the waiver will require little more than my signature because everbody knows I can't spell worth a darn. Let's just do and get it over with.
Brock, if these forms have been created but not yet printed let me know and I can run a couple hundred off and have ready and available for ACM8. Unless of course there is an online sign up and that would negate this need.

Shawn, it's handled, but thank you. As mentioned previously, we hope to have them available online for folks to print on their own (the individual/yearly option) before ACM. Either way we will have them available day of.
Printed 103 just in case...hell even signed one myself.
Okay, in order to make this as painless as possible—and we do realize it's a hassle and we hate to have to act like grown-ups as much as everyone else does—we have a yearly waiver and release of liability that people can download below. Print it, sign it, bring it to ACM and you can forget about it for the rest of the year.

"Page not found"
If you get a "page not found", try this:
Go to the Resources tab, the waiver is available there. It works on a couple computers I've used.
If you are still having an issue finding the form, it sounds like it's a local (to your computer) issue. On that, I'm out of my depth, but I'm sure others here have enought IT experience to have an idea.
Of course, there will be hard copies at ACM.
Nick--not sure why you are having problems. The form pops up for me both when I am logged in to DOD and when I am "anonymous". I've tried it on two different browsers with no issues. Can you try the link again?
I hadn't tried it since the intitial test, but now it seems to work.
I'll make a goal to print and sign it before the night is over!
